Case Management
At Med-Law Connection, we take an "outcome -
based approach" to provide quality, cost effective
healthcare for those patients whose
circumstances require a case manager to
oversee their needs.
Our case managers
position themselves where the needs of the
patient, the health care team and the insurance
carrier intersect. The outcome we always strive
for is improved quality of life for the patient.
To achieve that outcome, a Med-Law
Connection case manager watches over every
phase of care and rehabilitation.
Throughout that process, our case
manager acts to:
- Identify Risks
- Identify and prevent possible medical complications
- Prevent accidents
- Promote the patient's independence and integration into the community
A key role involves teaching the patient to
participate in his or her treatment plan. Showing
the patient the long-term result of a care
decision can make a huge difference. Spending
now for appropriate, quality care can mean the
saving of future costs caused by complications or even greater disabilty.
