Case Management

At Med-Law Connection, we take an “outcome – based approach” to provide quality, cost effective healthcare for those patients whose circumstances require a case manager to oversee their needs.

Our case managers position themselves where the needs of the patient, the health care team and the insurance carrier intersect. The outcome we always strive for is improved quality of life for the patient. To achieve that outcome, a Med-Law Connection case manager watches over every phase of care and rehabilitation. Throughout that process, our case manager acts to:

  • Identify Risks
  • Identify and prevent possible medical complications
  • Prevent accidents
  • Promote the patient’s independence and integration into the community

A key role involves teaching the patient to participate in his or her treatment plan. Showing the patient the long-term result of a care decision can make a huge difference. Spending now for appropriate, quality care can mean the saving of future costs caused by complications or even greater disabilty.

Services Provided

  • Coordinate medical treatment at the onset of injury to ensure positive outcomes resulting to a quicker and less costly recovery
  • Access current treatment plans, identify medical needs based on objective findings, and monitor progress
  • Determine anticipated return to work date, verify medical restrictions and capabilities, and obtain maximum medical improvement
  • Evaluate future medical expenses
  • Cases requiring medical file analysis, provide chronology development and documentation
  • Assessing multiple health problems unrelated to the work injury yet interfering with return to work
  • Assist multiple injury types, spinal conditions, surgical cases, head trauma, and multiple conditions affecting the progress of the client
  • To help determine causality, diagnosis and causal medical relevance to the injury
  • When social and psychological factors influence recovery
  • Hospitalization coordination; discharge planning; and facilitate/assess Home Care
  • Coordination of Independent Medical Exams, Consultations, and diagnostic services with experts in the field
  • Coordination of Vocational Evaluations
  • Establish treatment requirements that will enable claimant to gainful employment

Red Flag Indicators

  • Relationship of accident to injury is unclear
  • Medical guidelines indicate the treatment may be excessive
  • Treatment plan is not clear or definitive
  • Lost time over 30 days
  • Indications for an Independent Medical Examination (IME)
  • Multiple medical conditions with multiple diagnoses exist, treatment is unusual or excessive
  • Multiple medical providers and ancillary services
  • Possibility of pre-existing conditions
  • Delay in return to work
  • Medical and non-medical issues seem to complicate recovery
  • Recovery slow without medical indication
  • Medical treatment does not correlate with accident, injury or disability facts
  • Treating providers have failed to provide medical information or clarification despite requests
  • Failure to follow the medical treatment plan, unwillingness to communicate
  • Unusual medical conditions

Elder & Geriatric Assesment

Med-Law Connection can assist with:

  • Physician liaison: communicate with the physician(s) to coordinate care.
  • Geriatric assessment: initial and ongoing clinical assessment to assess clients status related to medical condition, ADL’s (activities of daily living), living arrangements and medication management.
  • Management of Medical and custodial care needs by facilitating appropriate community referral.
  • Liaison between medical needs and financial administrator: assist with making sure medical needs are met and services/billing are appropriate to clients medical condition.
